Which plan is right for you?

BlueCare Plus (HMO D-SNP)℠

You may qualify for this plan if you:

Have Medicare Parts A and B
Are enrolled in BlueCare Tennessee Medicaid
Don't receive long-term services and supports (LTSS) or Employment Community First CHOICES

BlueCare Plus Choice (HMO D-SNP)℠

You may qualify for this plan if you:

Have Medicare Parts A and B
Are enrolled in BlueCare Tennessee Medicaid
Receive long-term services and supports (LTSS) or Employment and Community First CHOICES

You may qualify for this plan if you:

Have Medicare Parts A and B
Have Qualified Medicare Beneficiary (QMB) status
