BlueCare Plus (HMO D-SNP)℠

This plan is for BlueCare℠ members who qualify for Medicare. You might be eligible for this plan if you’re not part of TennCare’s LTSS CHOICES or Employment and Community First CHOICES programs.

This plan includes:

$0 Premiums and deductibles
$0 Copays for preferred generic drugs
Monthly allowance for healthy food , over-the-counter items and rides to plan-approved locations through Uber Health

BlueCare Plus Choice (HMO D-SNP)℠

This plan is for BlueCare members who qualify for Medicare and are enrolled in TennCare’s LTSS CHOICES or Employment and Community First CHOICES programs.

This plan includes:

$0 Premiums and deductibles
$0 Copays for preferred generic drugs
Monthly allowance for healthy food , over-the-counter items and rides to plan-approved locations through Uber Health

BlueCare Plus Select (HMO D-SNP)℠

This plan is for people in TennCare’s Qualified Medicare Beneficiary (QMB) program. It’s also for people who qualify for Medicare and are in a TennCare 1915(c) waiver program.

This plan includes:

$0 Premiums and deductibles
$0 Copays for preferred generic drugs
Monthly allowance for healthy food , over-the-counter items and rides to plan-approved locations through Uber Health
