Plans and Eligibility
BlueCare Plus Tennessee is an HMO D-SNP with a Medicare contract and a contract with Tennessee Medicaid (TennCare). Enrollment in BlueCare Plus Tennessee depends on contract renewal.
BlueCare Plus Tennessee plans are for those with Medicare and Medicaid assistance. For BlueCare PlusSM and BlueCare Plus SelectSM, the Medicaid categories we accept are Qualified Medicare Beneficiary (QMB) only, QMB+, Specified Low Income Beneficiary Plus (SLMB+) and Full Benefit Dual Eligible (FBDE).
For BlueCare Plus ChoiceSM, the Medicaid categories we accept are QMB+, SLMB+ and FBDE. You must also be a BlueCare Choices 1, 2 or 3 member.
By enrolling in a BlueCare Plus Tennessee plan, eligible members will still receive TennCare and Medicare benefits. Medicare benefits will be covered by BlueCare Plus Tennessee and Medicaid benefits will continue to be covered by Medicaid or your TennCare plan.
Any benefit costs shown are for providers in our network. We provide members with a flex card to use for select plan benefits. Value of the flex card is preloaded with certain amounts, according to benefits. The allowances for healthy food and housing utilities are part of a special supplemental program for people living with chronic health conditions. Not all members qualify. Value of card may be zero. Card may not be used at all merchants or providers. Merchants and providers must accept major credit cards. Purchases may be restricted to certain types of items and services.

Natural disasters are unpredictable. But as a BlueCare Plus Tennessee plan member, you can count on getting the care you need. If the governor of your state, the U.S. Secretary of Health and Human Services or the president of the U.S. declares a state of disaster or emergency in your area, you’ll still get care from your plan.
Do not wait — go to the nearest facility to get help. Your health and safety are the most important things. We’ll help take care of the rest. Generally, during a disaster, you can get care from providers who aren’t in our network for the in-network cost-sharing amount. If you can’t use a network pharmacy during a disaster, you may be able to fill your prescription drugs at an out-of-network pharmacy too.
