Why is this happening?
The Centers for Medicare & Medicaid Services is requiring this change to help make it easier for you and your providers to manage your care. When your D-SNP plan is the same as your TennCare health plan, they can work together to help you get the most from your benefits. You’ll have one place to call for help with all your benefits. Your providers will have one place to call as well.
What happens if you don’t make any changes?
If your D-SNP plan and your TennCare plan don’t match by Jan. 1, 2027, you’ll be disenrolled from your D-SNP and moved to Original Medicare. This means you won’t have a prescription drug plan. You’ll need to enroll in one yourself, or Medicare will automatically enroll you in a plan.

How to align your plans
There are two ways to make your Medicare D-SNP and TennCare health plans match.
If you want to stay with your TennCare health plan, you’ll need to change your Medicare D-SNP plan.
- There’s a monthly Special Enrollment Period for people with both Medicare and Medicaid. You can move into a fully integrated D-SNP plan once each month. That allows you to match your D-SNP plan to your TennCare health plan.
- You can also change your D-SNP plan to match your TennCare plan during Medicare’s Annual Enrollment Period (Oct. 15 through Dec. 7).
- For help with changing your D-SNP plan, call Medicare at 800-MEDICARE (800-633-4277). You can call 24 hours a day, seven days a week (except some federal holidays).
- You can also call us at 888-413-9637, TTY 711. From Oct. 1 to March 31, you can call us seven days a week from 8 a.m. to 9 p.m. ET. From April 1 to Sept. 30, you can call us Monday through Friday from 8 a.m. to 9 p.m. ET.
If you want to stay with your D-SNP plan, you’ll need to change your TennCare health plan.
- TennCare is offering a special enrollment period from June 1 through Oct. 31, 2026. If your D-SNP and TennCare plans don’t match, you can use this time to change your TennCare health plan to match your D-SNP plan.
- There are three ways to change your TennCare health plan during the special enrollment period:
